  • Your name, address, telephone number, and email, and a statement that you consent to the jurisdiction of the federal court in your district and that you will accept service of process from the person who provided the original DMCA notification

5. Repeat Infringers
DIGIRATE PRO reserves the right to terminate accounts or access to the Website for users who are found to be repeat infringers of copyright.
